rcu: Eliminate RCU_BH_FLAVOR and RCU_SCHED_FLAVOR

Now that the RCU flavors have been consolidated, RCU_BH_FLAVOR and
RCU_SCHED_FLAVOR are no longer used.  This commit therefore saves a
few lines by removing them.
