Added CREATE TEMPORARY TABLES and LOCK TABLES to db and host tables

Fixed bug in SELECT ... ORDER BY ... LIMIT
Fixed bug in ALTER TABLE and RENAME TABLE with --lower-case-table-names
Fixed hang when using --with-openssl
