USB keyspan driver

  
  general cleanups:
  	- removed commented out function prototypes
  	- removed duplicate function prototypes
  	- removed unneeded #include
  	- added named initializers
  	- merged lots of the individual device structures into general 1, 2, and 4 port structures.
