New improved IO_CACHE


include/my_global.h:
  Remove dbug_assert()  (One should use DBUG_ASSERT() instead)
mysql-test/mysql-test-run.sh:
  Fixed that xterm works on Solaris 2.8.
  Fixed printing of errors
