PCM Midlevel,RawMidi Midlevel,ALSA<-OSS emulation,ALSA sequencer
Fixed __user pointers including other misc fixes:
 - replaced the obsolete CONFIG_SND_IOCTL32_EMUL.
 - added the proper segment change before passing the kernel pointer
   as the user pointer in PCM code.
