- remove page_cache_entry - I think it never ever was actually used.
 - remmove wake_up_page (as in 2.4) - never used
 - remove misleading comment ontop of pagemap.h
 - make wait_on_page* directly call wait_on_page_bit
 - some reordering and additional comments in pagemap.h
