Pull module updates from Jessica Yu:

 - Harden CONFIG_STRICT_MODULE_RWX by rejecting any module that has
   SHF_WRITE|SHF_EXECINSTR sections

 - Remove and clean up nested #ifdefs, as it makes code hard to read
