ALSA update

  - fixed compilation problems for rawmidi, sb16, ac97_codec, ens1370
  - ES1968 - added use_pm parameter
  - USB audio
    - complete callback follows 2.5
    - fixed support for FU and PU controls (mixer)
    - updated SB Extigy comments
