xen: add /proc/xen/xsd_{kva,port} to xenfs

These are used by the userspace xenstore daemon, which runs in dom0.
Xenstored is what's behind the xenfs "xenbus" filesystem.

[ Impact: provide mapping and port to usermode for xenstore ]
