tracing: rename DEFINE_TRACE_FMT to just TRACE_FORMAT

There's been a bit confusion to whether DEFINE/DECLARE_TRACE_FMT should
be a DEFINE or a DECLARE. Ingo Molnar suggested simply calling it
TRACE_FORMAT.
