[PATCH] remove clustered_apic_mode from smpboot.c

This removes clustered_apic_mode from smpboot.c into subarch,
creating a headerfile "mach_wakecpu" for all the cpu wakeup stuff.
This is pretty much the last of clustered_apic_mode ... ;-)
