Code had hardcoded limits to the maximum stream that could be used, 
and consequent static data structures.   Now dynamically allocate
storage for the SSN maps until _after_ we know what they are.  
Protocols such as SIP want to use all possible streams.

This fixes a potential problem in the migration thread for the case that
the first CPU in the cpus_allowed mask of a process is offline.

This fixes the 'iret' exception recovery code to use the new ES/DS segment
rules - load them with __USER_DS instead of __KERNEL_DS.

It also fixes a typo in a comment.
